Our Community Health Workers assist seniors, individuals and families in our community access resources that improve their physical and mental health and quality of life. We make the important connections between people seeking assistance and the resources that can answer questions and provide care. We also provide social support, informal counseling and work on a one-to-one basis.

We look at everything that is getting in the way of good health outcomes, access to health care, housing, employment, and education, because it all matters.
